Demography Beyond Age: Understanding Sex, Gender Identity, and Ethnicity

My work explores how we can sharpen our thinking and performance in the demanding realities of modern clinical practice.

Demographic information often provides our first glimpse into the circumstances, experiences, pressures and risks that shape a patient’s life.

Used thoughtfully, it offers valuable context, helping us understand not only what may be happening medically but also how best to communicate, support decision-making and tailor care to the individual sitting in front of us.

In my previous post, The Personal Side of Demography: Age Beyond the Statistics, I explored why age is one of the most influential factors in clinical reasoning. Yet age alone never tells the whole story.

Every patient is one person, but viewed through many different facets.

To truly understand the person behind the illness, we also need to consider other demographic characteristics—biological sex, gender identity, gender expression, and ethnicity—and examine how they influence consultation, clinical reasoning, and patient care.

In this post, we’ll explore how these characteristics help us better understand our patients and deliver more personalised care.

What Do We Mean by Biological Sex, Gender Identity, Gender Expression and Ethnicity?

Biological sex, gender identity and gender expression are related yet distinct concepts.

  • Biological sex refers to the anatomical and physiological characteristics present at birth. Most people are born male or female, although some are born with variations in sex characteristics (intersex) or with atypical sex chromosomes.
  • Gender identity is a person’s internal sense of who they are—whether male, female, transgender, non-binary, gender-diverse, or another identity.
  • Gender expression reflects how a person presents themselves to the world.

For most people, gender expression reflects their gender identity, which in turn aligns with their biological sex.

Ethnicity denotes a shared cultural identity rooted in common ancestry, history, language, religion, traditions or society. It differs from race, which classifies people by physical characteristics, and from nationality, which refers to legal citizenship.

Although these characteristics are often recorded as routine demographic information, they have important clinical relevance when interpreted thoughtfully and in the appropriate context.

Why These Characteristics Matter

Biological sex, gender identity, gender expression and ethnicity are far more than details recorded in a patient’s notes. Together, they offer valuable insight into who our patients are, the experiences that have shaped them and the context in which illness develops.

They may influence:

  • Disease risk and prevalence.
  • Healthcare needs.
  • Communication and shared decision-making.
  • Cultural beliefs and expectations.
  • Socioeconomic circumstances.
  • Access to healthcare.

When considered thoughtfully, these characteristics help us build rapport, broaden our diagnostic thinking and tailor investigations, treatment and follow-up to the individual rather than simply treating the presenting condition.

The aim is not to categorise patients but to understand the clinical relevance of these characteristics for the person in front of us.

How These Characteristics Shape the Consultation

Demographic characteristics may influence how a consultation is conducted or how information is interpreted. However, not every detail needs to be explored in every consultation.

The key is relevance.

During the introductory conversation, establish how the patient wishes to be addressed.

  • What name would you like me to use?
  • What pronouns do you prefer?

Where clinically relevant, establish biological sex, together with any relevant aspects of gender identity and ethnicity, respectfully and sensitively.

Throughout the consultation, remain mindful of the risks of assumptions, stereotyping and unconscious bias. Equally, if you feel your background, gender or ethnicity might affect a patient’s comfort or trust, asking a simple, open question may help.

  • Do you feel comfortable talking to me today?
  • Is there anything you’d like to discuss before we begin?
  • Is there anything that would help you feel more at ease?

From a clinical perspective, biological sex remains important because anatomical and physiological differences influence disease prevalence, clinical presentation, examination, investigation and treatment.

Similarly, aspects of gender-affirming care—such as hormone therapy or previous surgery—may sometimes have important clinical implications.

Understanding a patient’s ethnicity can also be clinically relevant, as it may provide insight into socioeconomic circumstances, cultural beliefs, health risks, and treatment preferences.

Approaching these conversations with curiosity, cultural humility and respect strengthens rapport, improves communication and supports shared decision-making. A tactful conversation and respectful examination, with a chaperone where appropriate, help to establish trust.

How These Characteristics Influence Diagnosis

Gender identity and gender expression are usually not diagnostically relevant, although aspects of gender-affirming care—such as hormone therapy or previous surgery—may influence diagnosis, investigation or treatment.

Biological sex and ethnicity, on the other hand, can significantly influence both disease probability and clinical presentation.

Like age, these characteristics influence probability rather than certainty. They help us think more clearly about what is likely, but they should never replace careful clinical reasoning. Every patient must still be assessed as an individual.

Biological Sex is Diagnostically Important

Biological sex remains an important part of clinical reasoning because some diseases occur only in people with specific reproductive anatomy, while many others differ in prevalence or presentation between males and females.

Examples include:

  • Prostate and testicular diseases occur only in people with male reproductive anatomy.
  • Endometriosis, uterine fibroids, polycystic ovary syndrome and ovarian cancer occur only in people with female reproductive anatomy.

Many other conditions vary substantially in prevalence.

For example:

  • Breast cancer is considerably more common in women.
  • Women are more susceptible to many autoimmune diseases.
  • Men have higher rates of abdominal aortic aneurysm and premature ischaemic heart disease.

Biological sex also influences the interpretation of laboratory investigations, including hormone concentrations, creatinine levels, and other reference ranges.

Ethnicity Influences Probability

Ethnicity may also alter the likelihood of particular conditions.

Examples include:

  • Sickle cell disease is more common among people of African, Caribbean, Mediterranean, Middle Eastern and South Asian ancestry.
  • Cystic fibrosis is more prevalent among people of Northern European ancestry.
  • Type 2 diabetes is more prevalent in several South Asian, Black African and Black Caribbean populations.
  • Crohn’s disease and ulcerative colitis have historically shown a higher prevalence among Jewish populations.

These associations should guide clinical reasoning, while recognising that individual patients may not align with population averages.

Population trends help us estimate probabilities, but they should never become assumptions about the individual sitting in front of us.

How These Characteristics Influence Management

Unlike biological sex and ethnicity, gender identity and gender expression typically do not directly affect management decisions.

Biological Sex

Biological sex influences management in several important ways.

For example:

  • When selecting investigations.
  • Considering pregnancy before requesting investigations involving ionising radiation
  • Recognising sex-specific screening programmes
  • Understanding differences in drug metabolism
  • Recognising differences in disease progression and treatment response.

Ethnicity

The influence of ethnicity on management is often more subtle.

Ethnicity contributes to both health equity and health disparities—the preventable differences in health outcomes that arise from unequal opportunities, unequal access to care, and broader social determinants of health.

For the individual patient, ethnicity may influence:

  • The choice of investigation
  • Interpretation of certain laboratory results
  • Pharmacogenomics and drug metabolism
  • Cultural beliefs and treatment preferences
  • Adherence to treatment.

Recognising the influence of biological sex and ethnicity promotes personalised care, ensuring equal care and consideration for everyone and preventing assumptions and unconscious bias.

Take-Home Message

Demography is personal.

As with age, biological sex, gender identity, gender expression and ethnicity are far more than statistics recorded in a patient’s notes. They provide valuable clues about the person behind the disease, helping us understand their risks, experiences, priorities and the wider circumstances that shape their health.

Demographic characteristics help us estimate probabilities and provide context, but they never define the individual in front of us, as every patient is unique. 

Understanding the patient as a person means looking beyond demographic labels to appreciate the experiences, values and circumstances that shape health and healthcare decisions.

Used thoughtfully, demographic information strengthens rapport, sharpens clinical reasoning, improves diagnostic accuracy, and supports more personalised care. 

Used carelessly, it can lead to assumptions, stereotyping and unconscious bias.

The challenge for healthcare professionals is not merely to collect demographic information but to interpret it wisely.
